54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Provided consultation on safe sleep practices; cribs when being used by infants shall have a tight fitting bottom cover; infants must be placed on their back to sleep; no soft bedding of any kind shall be used under or around infants; sleeping infants must be monitored either by in-person checks at least every 15 minutes, by using a baby monitor, or by placing the infant to sleep in a location that is within sight and hearing of the caregiver.

Comments:
An unannounced monitoring inspection was conducted from 9:30 - 11:30 am with the provider. There were seven children in care, ranging from nine months to three years of age, for a total of 18 points. The children were observed listening to a story, washing hands and eating snack, having free play, and sleeping (infants.) Snack served today: teddy grahams, yogurt raisins and water. Four children's records and two provider/household member records were reviewed. Required information was posted in addition to a variety of seasonal artwork. The last emergency escape drill was conducted: 11/1/19 and the last shelter-in-place drill was conducted: 12/6/19. The provider is Medication Administration Training (MAT) certified however there were no medications onsite to review. Violations:
Standard #: 22VAC40-111-90-A
Description: Based on a review of four children's records, the provider did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: Child #2 (date of first attendance: 11/4/19) did not have documentation of immunizations in their file.

Plan of Correction: Will ask the parent to provide documentation of immunizations.

Standard #: 22VAC40-111-520-C
Description: Based on observation, a crib in use had mesh sides. Evidence: Child #1 was observed sleeping in a bassinet with mesh sides.

Plan of Correction: All children will be put to sleep in cribs from now on.

Standard #: 22VAC40-111-570-A
Description: Based on observation, and provider interview, the provider failed to ensure that a caregiver does not exceed 16 points. Evidence: there were seven children in care: age 9 months, age 1 year, age 2 years, age 2 years, age 2 years, age 3 years and age 3 years for a total of 18 points.

Plan of Correction: Will have an additional provider this afternoon, and I will have an additional full-time provider beginning in January.
