- Technical Assistance:
Discussed the importance of crib safety - no linens or soft bedding should be placed around or inside of cribs.
An unannounced monitoring inspection was conducted on 12/16/2019 from 11:15 am - 1:15 pm.
At the time of entrance there were eight children in care with two caregivers. The ages of the children counted for a total of 22 points. Children were observed playing games in the play room with the assistant caregiver. Children also engaged in an interactive dance video.
Records were reviewed for two caregivers and five children. No medication is administered.
Violations were observed in three parts of the FDH standards and are listed on the violation notice.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider failed to ensure that children's records contain all of the required elements.
1. The enrollment record for child 2 lacked the following required information:
a. Home address, and telephone number of one parent;
b. Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: All missing information will be requested from the parent of child 1 today.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
1. The most recent immunizations on file for child 1, age 19 months, were administered on 10/15/2018.
2. The provider confirmed that updated immunizations were not on file for child 1.
Plan of Correction: The provider stated she would request current immunization information from the parent of child 1 at pick-up today.
Standard #: 22VAC40-111-530-B Description: Based on observation, the provider failed to ensure that cribs when being used by infants shall have a tight-fitting bottom cover.
1. The crib sheet in the crib used for napping by child 3, age 12 months, had a crib sheet that was loose fitting.
Plan of Correction: A tight fitting crib sheet will be placed on this crib immediately.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider failed to ensure that children's hands are washed with liquid soap and warm running water after diapering.
1. The provider did not wash the hands of child 1, age 19 months, after a diaper change at approximately 11:30 am.
a. Child 1 was taken directly to the playroom after the diaper change.
b. The provider did wash child 1's hands when reminded to do so by the inspector.
Plan of Correction: The provider stated that she used a wipe to clean the hands of child 1 while he was on the changing table.
Going forward children's hands will be washed in the bathroom with soap and water after a diaper change.
Standard #: 22VAC40-111-690-D Description: Based on observation, the provider failed to ensure that the diapering surface shall be cleaned and sanitized after each use.
1. After completion of a diaper change at approximately 11:30 am the provider did not clean the diapering surface with soap and water.
a. Additionally, the provider immediately wiped the diapering surface immediately after application of the sanitizing solution.
b. The definition of sanitizing states that the surface shall be allowed to air dry after application of the sanitizing solution.
Plan of Correction: The provider stated that the diapering surface will be cleaned with soap and water after each diaper change and that the sanitizing spray will be allowed to air dry.
Standard #: 22VAC40-111-690-E Description: Based on observation, the provider failed to ensure that soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
1. The provider placed the soiled diaper and wipes in a plastic grocery bag after completion of a diaper change at approximately 11:30 am.
a. The provider took the grocery bag out to the trash after hand washing.
Plan of Correction: The step down diaper pail will be used going forward.
