Comments:
An unannounced monitoring inspection was conducted today from 9:15am to 10:00am with the provider. There were no daycare children in care, only one of the provider's own children. The provider shared that she currently only has school age children enrolled and the three who had attended this morning had gone to school and would return after an early dismissal this afternoon. Three children?s records and one caregiver record were reviewed. The provider has current certification in CPR and First Aid. The provider does not administer medication. The emergency drill log was reviewed. The current week's menu was posted. Standard #: 22VAC40-111-830-C
Description: Based on documentation review and interview with the provider, the provider did not maintain documentation of evacuation drills as required. Evidence: The last documented evacuation drill was dated 9/30/19 but the provider stated she completed drills in the months of October, November, and December but she forgot to document them.

Plan of Correction: I will make sure I document all of my drills moving forward.

Standard #: 22VAC40-111-60-B
Description: Based on review of three children's records, the provider did not obtain all required documentation for each child's record. Evidence: 1. The record for Child A did not contain documentation of the name and policy number of the child's medical insurance. The records for Child B and Child C did not contain documentation of an annual review of the child's emergency contact information by the parents.

Plan of Correction: I will ask the parents for that information this afternoon.

Standard #: 22VAC40-111-180-A
Description: Based on review of one caregiver record, the provider did not obtain documentation of a current Tuberculosis (TB) test or screening every two years from the date of the first screening. Evidence: The most recent TB test on record for Caregiver A was dated 10/20/16.

Plan of Correction: I am almost certain I had a TB test since then, I will reach out to the office where I had it done to ask for a copy. If that is the most recent one, then I will obtain another one.
