Standard #: 22VAC40-111-830-C Description: Based on documentation review and interview with the provider, the provider did not maintain documentation of evacuation drills as required. Evidence: The last documented evacuation drill was dated 9/30/19 but the provider stated she completed drills in the months of October, November, and December but she forgot to document them. Plan of Correction: I will make sure I document all of my drills moving forward.
Standard #: 22VAC40-111-60-B Description: Based on review of three children's records, the provider did not obtain all required documentation for each child's record. Evidence: 1. The record for Child A did not contain documentation of the name and policy number of the child's medical insurance. The records for Child B and Child C did not contain documentation of an annual review of the child's emergency contact information by the parents. Plan of Correction: I will ask the parents for that information this afternoon.
Standard #: 22VAC40-111-180-A Description: Based on review of one caregiver record, the provider did not obtain documentation of a current Tuberculosis (TB) test or screening every two years from the date of the first screening. Evidence: The most recent TB test on record for Caregiver A was dated 10/20/16. Plan of Correction: I am almost certain I had a TB test since then, I will reach out to the office where I had it done to ask for a copy. If that is the most recent one, then I will obtain another one.
