Reminder: 22VAC40-111-(2)-60-B-14 - Documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B.

An unannounced monitoring inspection was conducted with the provider and assistant beginning at 11;20 AM and ending at 12:45 PM. There were nine daycare children and one resident child in care during the inspection. The children's ages were 7 months, 9 months, 21 months, 2, 2, 2, 2, 3, 3, 4 years old for a total of twenty-five points. The infants were observed doing tummy time and playing with soft toys. The older children were observed playing with blocks, dolls, and cars. Children were also observed preparing for lunch which consisted of chicken nuggets, fries, mixed vegetables, and fruit juice.Five children's records were reviewed. Two household records and in compliance. One pet record was reviewed and in compliance. There were no medications on site to be reviewed. The most recent emergency escape drill was documented as 12/5/19. The most recent shelter-in place drill was documented as 5/19/19.

Standard #: 22VAC40-111-50-A
Description: Based on a review of documentation, the provider did not ensure that children were accounted for on the daily written attendance record. Evidence: There were nine day care children and one resident child physically present but seven children recorded on the daily attendance sheet.

Plan of Correction: The daily attendance sheet was updated during the inspection. The provider and assistant will ensure that each child is recorded on the daily attendance sheet.

Standard #: 22VAC40-111-60-A
Description: Based on a review of documentation, not all of the children's records contained the required information. Evidence: Child A's record was missing the name and policy number of the child's medical insurance. Child B's record was missing the policy number for the child's medical insurance.

Plan of Correction: The provider will request the missing information from the parents as soon as possible.
