Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, January 6, 2020 from 11:05am to approximately 1:40pm to determine the provider's compliance with licensing standards at the family day home. During the inspection, there were four children present totaling 11 points with one provider. Upon the inspector's arrival, the provider and the children were observed in the main play area of the family day home. The licensing inspector observed the children engaged in play with their peers, singing songs, playing with toys, watching television, and interacting with the provider. The menu was posted and today's lunch consisted of tuna salad, cheese, bread, mixed vegetables, and milk. The licensing inspector interacted with the children when appropriate throughout the inspection. The provider was observed having positive interactions with the children in care. The required postings to include the license, emergency contacts, and recent inspection summary were inspected and found to be in compliance. The first aid kit and emergency supplies, designated childcare areas, the bathroom, and the kitchen were inspected. During the inspection, five children's records, one provider?s records, and two household members? records were reviewed. Transportation is not provided and medication not administered at this time. The last emergency evacuation drill was documented and conducted on December 18, 2019 and the last shelter-in-place drill was documented on September 20, 2019.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of five children's records on 1/6/2020, the provider did not ensure one child's record contained the required information.

Evidence: 1) The record for Child #1, enrolled on 1/3/2019, did not contain the telephone number for the two emergency contacts listed. Each child record shall contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. 2) The record for Child #1, enrolled on 1/3/2019, did not contain the policy number of the child's medical insurance.

Plan of Correction: The provider contacted the child's parent while the inspector was on site to obtain the missing information.

Standard #: 22VAC40-111-180-A
Description: Based on a review of records and interview on 1/6/2020, the provider did not obtain for each caregiver and adult household member a current report of tuberculosis (TB) screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence: 1) The TB screening results in the record for Provider #1 were dated 7/24/2017 and expired on 7/24/2019. 2) The TB screening results in the record for Household Member #1 were dated 7/24/2017 and expired on 7/24/2019. 3) The TB screening results in the record for Household Member #2 were dated 7/24/2017 and expired on 7/24/2019. 4) During interview, Provider #1 reported the most recent TB screening results for Provider #1, Household Member #1, and Household Member #2 could not be located.

Plan of Correction: The provider will locate the missing TB screening forms. If unable to locate, each person will obtain new TB screening results and will do so every two years.
