Technical Assistance:
Technical assistance was offered regarding immunization records for children in care.

Comments:
An unannounced monitoring inspection was conducted from 10:25 am through 11:40 am. There were 10 children (27 points) with 2 caregivers present. A total of 5 children's records, 2 caregiver's records and 1 household member's record was reviewed. The children were engaged in the following: toy play, bathroom breaks, hand washing, and eating lunch. The lunch menu included chicken breast with tortellini, pears, spinach and milk. Standard #: 22VAC40-111-710
Description: Based on observation, prescription medication was not maintained in in the original and labeled container.
Evidence: Medications for Child # 2 and Child # 3 did not have the prescription label and were not labeled with the children's names.

Plan of Correction: I will ask the parents to bring in the prescription label and make sure the medications are labeled.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain subsequent tuberculosis (TB) screenings for household members every two years.
Evidence: Household Member # 1's most recent TB screening expired on 8/12/19. An updated TB screening was not obtained.

Plan of Correction: An updated TB will be obtained this week.

Standard #: 22VAC40-111-540-B
Description: Based on observation, an infant walker was being used in the family day home.
Evidence: Child # 1 (6 months old) was observed using an infant walker.

Plan of Correction: The walker will no longer be used.
