Standard #: 22VAC40-111-100-A Description: Based on review of five children's records, the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: The record for child #5 (first date of attendance 9-10-18) did not include documentation of a physical.Plan of Correction: The provider will try to get it by the end of the week and put it in the record.
Standard #: 22VAC40-111-500-C Description: Based on observation and measurements and provider interviews, the provider did not ensure that stationary outdoor playground equipment shall meet all requirements.
Evidence: In the designated outdoor play space there was a metal swing set with an attached plastic slide that was within 2 1/2 to 3 1/2 feet of a plastic green/red slide. The equipment was not placed at least six feet from the perimeter of other play structures or obstacles.Plan of Correction: The provider will have the green/red slide moved it today.
