Comments:
An unannounced monitoring inspection was conducted on 01/09/2020. There were 3 children, ages 3 years-4 years, totaling 6 points and 1 caregiver present during the inspection. The inspector reviewed 5 children's records, 2 caregiver records and 2 pet records during the inspection. The children were observed in the following activities: during free choice play, during story time, during an art project, and during lunch. The inspector reviewed the emergency supplies during the inspection. The inspector discussed attendance records and annual updates with the provider. The inspector arrived for the inspection at 10: 15 a.m. and departed at 12:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observations and discussion, the provider failed to ensure that the first aid kit contain the minimum requirements as required by the standards.

Evidence: The first aid kit did not contain a digital thermometer.

Plan of Correction: The provider will purchase a new thermometer.

Standard #: 22VAC40-111-60-B
Description: REPEAT VIOLATION

Based on record record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements required by the standards.

Evidence:
1. The record for Child #1 did not contain the office address and telephone number of the child's physician.
2. The record for Child #2 did not contain the address and telephone number of each custodial parent's place of employment, an employer was provided for both parents however there was not an address or telephone number provided.
3. The record for Child #3 did not contain the documentation of the review of the child's emergency contact information as required by 22VAC40-111-780-B; the most recent update was dated 10/01/2018.

Plan of Correction: The provider will get with the parents.

Standard #: 22VAC40-111-210-A
Description: Based on record review and discussion, the provider failed to ensure that in addition to satisfactory completion of first aid and CPR training, caregivers shall obtain a minimum of 16 clock hours of training annually relevant to their job responsibilities.

Evidence: The record for Caregiver #2 only contained documentation of 14.5 annual training hours.

Plan of Correction: Will do online courses to get the required training hours.

Standard #: 22VAC40-111-320-A
Description: Based on observations and discussion, the provider failed to ensure that small electrical appliances such as but not limited to, curling irons, toasters, blenders, can openers and irons shall be unplugged unless being used by the caregiver.

Evidence: The toaster and coffee maker were plugged in during the 01/09/2020 inspection.

Plan of Correction: The provider will keep the appliances unplugged when not in use.
