Technical Assistance:
Technical assistance was offered regarding First Aid and CPR training, and annual training hours.

Comments:
An unannounced monitoring inspection was conducted from 10:40 am through 12:25 pm. There were 8 children (24 points) with 2 caregivers present. A total of 5 children's files and 3 caregiver's files were reviewed. The children were engaged in the following: playing dress up, playing with blocks, bathroom breaks, hand washing, and lunch. The lunch menu included chicken, mixed vegetables, blueberries, sweet potato and milk. There were no medications on site. Emergency drill records were reviewed. Standard #: 22VAC40-111-830-B
Description: Based on record review, the family day home did not practice two shelter in place procedures, annually.
Evidence: The family day home's last shelter in place drill was conducted on 5/23/19.

Plan of Correction: I will complete and document two shelter in place drills annually.

Standard #: 22VAC40-111-60-B
Description: Based on records review, children's records did not have complete information.
Evidence:
1. Child # 1's record was last updated on 10/15/18. An annual update was not completed for the record.
2. Child # 2' record was missing information for 2 emergency contacts.
3. Child # 3's record was missing the provisions for emergency plan, information for persons authorized to pick up, first day of attendance, and the medical insurance policy number.

Plan of Correction: All the missing information will be obtained.

Standard #: 22VAC40-111-320-A
Description: Based on observation, small appliances were not unplugged when not in use.
Evidence: An electric kettle and a toaster oven were found to be plugged in while not in use.

Plan of Correction: The appliances were unplugged during the inspection.

Standard #: 22VAC40-111-540-A
Description: Based on observation, high chairs were not used according to manufacturer's instruction, and safety straps were not not used while occupied by children.
Evidence:
1. Three children were observed seated in high chairs.
2. Safety straps were not being used for two of the three children.
3. The third high chair did not have any safety straps.

Plan of Correction: The high chair without the safety straps was no longer being used. The child was removed to another high chair. All three children were seated in high chairs using the safety straps, during the inspection.
