Comments:

An unannounced monitoring inspection was completed on 1/15/2020. There were four children in care from two months through three years with the provider and assistant supervising. The point total was 12. There were four resident children also present with their caretakers present at the home. Five children's records were reviewed. The provider and assistant caregiver's records were reviewed for annual training updates only. The inspector arrived at the provider's home at 10:00 am and departed at 1:30 pm. The children were observed during free play time, during TV time, during lunch, and during music and movement activities. All infant activities were also observed.

Standard #:	22VAC40-111-900-A
Description:	Based on observation, the provider failed to ensure that to assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn was not served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise. Evidence: 1. The lunch that included whole grapes, whole raw baby carrots, and whole hot dogs were served and consumed by children ages two and three years of age. Whole grapes and whole raw baby carrots were also served to a 15 month old child.
Plan of Correction:	The provider cut the grapes and carrots when the inspector reviewed the choking hazard potential during the inspection. The hot dogs had been consumed by the two and three year old children. The provider will make sure foods that are potential choking hazards served to children under four years of age are prepared in a manner to reduce the risk of choking.

Description:	Based on record review, the provider failed to ensure that all required information was documented in each child's record. Evidence: 1. The records for children #3 and #4 (siblings) were missing documentation of the first date of attendance, allergies, and chronic physical problems/pertinent developmental information, if any. The spaces for allergy and physical/developmental information were left blank on the forms for that information. 2.The record for child #1 was missing documentation of a first date of attendance.
Plan of Correction:	The provider will have all information documented in the children's records.
