Technical Assistance:

Renewal of the MAT training was discussed with the provider. Also updating of a soon to be expiring tuberculosis screening was discussed. The requirement to keep all children's records for a minimum of two years following termination of care was discussed and the provider was reminded to always enter the child's first and last dates of attending in the child's record.

Comments:

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:40 PM an departed at 2:40 PM. Four children, totaling nine points were in care with the provider. Nap time was observed. Five children's records were reviewed. It was reported that no medication has been administered to children in care and no children's injuries were reported. An exit meeting was conducted with the provider prior to closure of the inspection.

Description:	Based upon review of five children's records, the provider has not ensured that each child's record includes documentation of a physical examination by a physician within 30 days after attending. Evidence: 1. The record provided for child 5, who began attending on 12/9/2019 , did not include documentation of a physical examination. 2. The provider verified that the record for child 5 did not include documentation of a physical examination.
Plan of Correction:	The provider responded with the following: Documentation of a physical examination will be obtained for child 5. In the future, documentation of a physical examination will be in each child's record prior to attending or within 30 days after attending.
