Comments:
A Subsidy Health and Safety inspection was conducted on 01/21/2020 was conducted from 10:30 am-11:30 am to review supplemental health and safety requirements. At the time of inspector's arrival there were 5 children, aged 15 months to 3 years, for a total of 12 points, in care with the provider and assistant provider. The records sample size consisted of 5 children's records and 2 provider / household member records. The child was observed engaging in creative play, and interacting with the provider. The provider stated that no medication is administered to children while in her care and there have been no injuries. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements are detailed in the Violation Notice and were discussed with the provider at the conclusion of this inspection.

Description: Based on record review and interview it was determined the provider did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occured.
Evidence:
1. The daily attendance did not document the arrival and departure of each child in care.
2. The provider confirmed that the arrival and departure was not documented on the daily attendance.

Plan of Correction: Provider will document the arrival and departure of each child on the daily attendance.
