- Technical Assistance:
An unannounced renewal inspection was completed on Thursday, January 9, 2020 from approximately 10:54am to 12:32pm to determine the provider's compliance with licensing standards. During the inspection, there were 8 children present totaling 23 points in the care of three caregivers. Upon arrival, the children were observed in the designated area for childcare watching television. Later, the children were observed preparing for lunch, washing their hands, and taking bathroom breaks. The caregivers were observed having positive interactions with the children. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys and materials to meet the needs of the children. During the inspection, the licensing inspector interacted with staff and children when appropriate. The bathroom, kitchen, and designated childcare areas were all inspected during today's inspection. The inspector observed the required postings. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were inspected. The outdoor space was inspected and found to be in compliance. Five children's records, five caregiver records, and one household member's record were reviewed today. There were no injury reports to review. The provider does not administer medications or provide transportation at this time. The most recent evacuation drill was documented on 12/16/2019. Two shelter-in-place drills were documented on 1/07/2019 and 11/14/2019.

Standard #: 22VAC40-111-30-A Description: Based on observation and interview on 01/09/2020, the provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.
Evidence: 1) The licensing inspector observed that the provider's zoning permit requires a conditional use permit for any family day home that employs persons from outside the home. 2) During interview, Caregiver #1 reported she does not have a conditional use permit from zoning to authorize the provider to employ a caregiver who is not a resident of the home. Caregiver #1 acknowledged six out of seven assistant caregivers do not reside in the home. 3) During the inspection, the licensing inspector observed two assistant caregivers that do not reside in the home.
Plan of Correction: The provider will contact zoning to obtain more information regarding a conditional use permit to be able to employ a caregiver or volunteer who resides outside of the home.
Standard #: 22VAC40-111-90-C Description: Based on a review of five children's records and interview, the provider did not obtain documentation of additional immunizations for one child who is not exempt from the immunization requirements.
Evidence: 1) The immunization documentation in the record for Child #5, two-year-old, was dated 8/2/2018. There were no additional immunizations since the child was enrolled on 8/6/2018. 2) During interview, Caregiver #1 reported no additional immunizations had been obtained for Child #5. The family day home shall obtain documentation of additional immunizations once every six months for children under the age of two years.
Plan of Correction: The provider will request additional immunizations from the child's parent today.
Standard #: 22VAC40-111-170-B Description: Based on a review of records on 01/09/2020, the provider did not ensure that two caregivers had a report of tuberculosis (TB) screening completed within the required time frame.
Evidence: 1) The TB screening in the record for Provider #2 (DOH: 11/22/2019) was dated 2/9/2018. 2) The TB screening in the record for Provider #5 (DOH: 11/18/2019) was dated 8/22/2019. The form shall have been completed within the last 30 days and be signed by a physician, physicians' designee, or an official of the local health department.
Plan of Correction: The provider will ensure TB screening results are completed within the 30 days prior to hiring a new employee.
