Technical Assistance:

As discussed the two slides on your background playground equipment is prohibited for use during the operating hours of the family day home due to the height of the climbing apparatus As discussed the pack and play is only to be used for play and not for sleeping.

Comments:

An unannounced Renewal inspection was conducted this date. A total of 11 children with Provider and 1 assistant present. When the Inspector arrived, the children were playing in small groups on the floor with toys, books and kitchen play sets. Artwork on display where children completed mosaic artwork using tissue paper and forming the letter "B". Walk thru of the entire home, playground in the backyard, crib room and bathroom was conducted. A total of 9 children's records, all staff and household records were reviewed. No medication needs at this time. The children transitioned to lunch after hand washing. After lunch, the children were singing in spanish and dancing in the program area. Description:	Based on child record review, 3 out of 9 records reviewed were incomplete. Evidence: Child #2's record did not include their first date of attendance, the proof of identity did not include the date in which provider reviewed the document and the provider's medication policy and liability insurance declaration was not completed. Child'3's record did not include the medication insurance policy number. Child#5's record did not include their first date of attendance.
Plan of Correction:	I have these records aside and will complete the documents with the parents at pick up today.
