63.2 Licensure and Registration Procedures

Technical Assistance:
As discussed the provider may contact Terry Gandy at 540-347-6345 to enroll in the phase 2 training located at the Warrenton office.

Comments:
An unannounced renewal study was conducted from 9:00 a.m. - 10:45 a.m. There were five children in care with the provider and one household member who was assisting. The ages of children in care were 6 months, 14 months, 18 months, 18 months, and 2 years for a total of 16 points. The children were observed participating in structured center play and eating a morning snack. Morning snack served today is crackers, raisins, and water. Five children's records were reviewed and in compliance. The provider and two household member records were reviewed. The most recent emergency escape drill was conducted 01/13/2020 and the most recent shelter in place drill was conducted 08/05/2019. Violations:
Standard #: 22VAC40-111-900-A
Description: Based on observation of morning snack it was determined that children under the age of 4 years old were served food that is considered a potential choking hazard. Evidence: Child B (Age 2), Child C, (Age 18 months), and Child D (Age 18 months) were served raisins with morning snack.

Plan of Correction: Raisins will no longer be served to children under the age of four years old.

Standard #: 22VAC40-111-180-A
Description: Based on record review of the provider and household members it was determined that not all household member records contained documentation of a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for household member A was dated 09/19/2017.

Plan of Correction: Household member A will obtain a new TB screening and submit the results to the provider.

Standard #: 22VAC40-111-280
Description: Based on observation it was determined that not all potentially poisonous substances were kept locked and inaccessible to children. Evidence: A container of ointment and a container of disinfecting wipes were observed unlocked on a toy shelf.

Plan of Correction: The ointment and wipes were locked during the inspection.

Standard #: 22VAC40-111-540-A
Description: Based on observation of the family day home it was determined that children were not always securely fastened with a safety strap while using high chairs. Evidence: Child A and Child B were observed sitting in high chairs and were not secured with the safety straps.

Plan of Correction: Children will be secured with safety straps while using the high chairs.

Standard #: 63.2-1720.1-B-3
Description: Based on review of the provider and household member's records it was determined that not all household member records contained documentation of a determination letter based on a fingerprint check before reporting to the family day home. Evidence: Household Member B who was also assisting in the family day home does not have documentation of a fingerprint determination later.

Plan of Correction: Household Member B will obtain a fingerprint check and submit the results to the provider. The provider will forward the results to the department for verification before the renewal is submitted for approval.
