Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector was in the facility between 10:30 AM and 12:30 PM. Seven children were in care with two caregivers. Morning learning activities, art project and choice activities were observed. A sample size of five caregiver/household records and five children's records were reviewed. It was reported that no medication is administered and no children's injuries were reported.

The Certificate of Occupancy from the City of Portsmouth indicates a maximum capacity of ten children. The license will be changed to reflect a new capacity of ten children.
The family day home is now a Limited Liability Corporation.

Please forward copies of the Certificate of occupancy and certificate of corporation to the inspector.

An exit meeting was conducted with the provider prior to closure of the inspection.
