The licensing inspector conducted an unannounced monitoring inspection on Tuesday, February 4, 2020 between 10:20 am to 12:00 pm to determine the provider's compliance with the licensing standards. During this inspection, there were three children in care totaling 8 points with one caregiver. The family day home designated childcare areas were inspected. The providers record and three children's record were reviewed during his inspection. The family day home's designated childcare areas, required posting, and emergency supplies were inspected. Description: Based on interview, the provider did not ensure to provide the menu for the current one-week period.

Evidence: Interview with provider confirmed a menu for the current week period was not complete.

Plan of Correction: The provider completed the menu during the inspection.

Standard #: 22VAC40-111-50-A
Description: Based on interview, the provider did not ensure to keep a written record of children in attendance each day.

Evidence: Interview with provider revealed a written record of children in attendance each day was not completed and available for review during the inspection.

Plan of Correction: The provider will ensure attendance is completed each day. Corrected during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's record and interview, the provider did not ensure all children's record contain the following information.

Evidence: In Child#3's record (start date 1/29/2020), the emergency contact information and parent agreement information was not available for review. Interview with the provider confirmed the information was not available during the inspection.

Plan of Correction: The provider will obtain copy of information today.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview, the provider did not ensure a landline telephone was accessible during the family day home.

Evidence: There was no landline telephone accessible during the inspection. Interview with provider confirmed a landline was not operable or accessible during the inspection.

Plan of Correction: The provider will follow up with company to have landline restored in two weeks.
