63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted on 02/11/2020. There were 6 children, ages 11 months-4 years, totaling 14 points and one provider present during the inspection. The inspector reviewed 5 children's records, one provider record, and one pet record during the inspection. The provider stated that medication is not administered. The children were observed in the following activities: watching an educational video, in free choice play, diapering and during lunch. The inspector reviewed the emergency supplies during the inspection. The inspector discussed hiring an assistant with the provider and reminded the provider of upcoming updates to children?s records. Standard #: 22VAC40-111-920 Description: Based on observations and discussion, the provided failed to ensure that when meals or snacks are provided by the family day home the menu for the current one-week period shall be dated, posted or placed in an area accessible to parents,list any substituted food, and be kept on file one week at the family day home.
Evidence: The menu for the current week of 02/10/2020-02/14/2020 was not posted upon arrival; the current weekly menu was posted during the inspection. The menu items listed for lunch on 02/11/2020 were not served, substitute items were served and were not noted on the weekly menu.
Plan of Correction: The menu will be posted and changed in advance.
Standard #: 22VAC40-111-690-A Description: Based on observations and discussion, the provider failed to ensure that a child shall not be left unattended on a changing table during diapering.
Evidence: Child #3 was observed being left unattended on 5 different occasions during diapering. The safety strap was secured and the provided walked to the bathroom on 5 occasions to wash hands and to dispose of various trash items.
Plan of Correction: The trash can will be moved closer to the changing table.
Standard #: 63.2-1720.1-A Description: Based on record review and discussion, the provider failed to ensure that all applicants shall undergo a background check in accordance with the Background Check Regulations prior to employment and every five years thereafter.
Evidence: The most recent search of the central registry in the record for Provider #1 was dated 02/10/2015; there was not a 2020 follow-up search of the central registry available for review during the 02/11/2020 inspection.
Plan of Correction: The provider will get that sent off by Friday.
