Comments:
An unannounced renewal inspection was conducted today between 9:00 am and 12:15 pm. There were 8 children (19 points) present with 2 caregivers. The children were observed eating breakfast, singing songs and playing musical instruments and working on art projects. Breakfast consisted of apples, bananas, grapes, pancakes and milk. The children were also observed during diaper changing and hand-washing.The entire physical plant, children's records, caregiver records, household member records, evacuation plans, menu's, cribs, evacuation drill logs, attendance logs and emergency supplies were inspected. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of physical examinations by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #2 and #3's records do not contain documentation of physical examinations by the 30th day of attendance.

Plan of Correction: I will get information from parent.

Standard #: 22VAC40-111-120-A
Description: Based on a review of caregiver records, the provider failed to ensure that all caregivers records contained required documentation.

Evidence:
Caregiver #2's records did not contain documentation of employment date, caregiver address, verification of age, two references, qualification of education or documentation of orientation.

Plan of Correction: Inspector sent provider Documentation of Assistant Orientation Form. Provider states she will fill out form and place in assistants file.

Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, the provider failed to ensure that children's records are maintained and contain up to date documentation.

Evidence:
1. Child #2 and #3's records do not have documentation of the name, address and telephone number of mother's place of employment, first day of attendance or the name of persons other than the parents who are authorized to pick up the child.

Plan of Correction: I will get the information from parent and place in file.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized by the first day of attendance.

Evidence:
Child #2 and #3's records did not have documentation of immunizations on the first day of attendance.

Plan of Correction: I will get information from parent.

Standard #: 22VAC40-665-250-A
Description: Based on observattion potentially poisonous substances and supplies are accessible to children.

Evidence:
Cleaning agents and personal items were unlocked in the bathroom making them accessible to children.

Plan of Correction: I will place a lock on the cabinet in the bathroom.
