Comments:
An unannounced monitoring inspection was conducted on February 13,2020. At the time of entrance there was 1 provider and 8 children with a total of 11 points. The sample size consisted of 5 children's records and 1 provider record. Children were observed working on valentine cards and eating lunch. The provider does not administer medications. The inspection began at approximately 11:00 am and ended at 1:00 pm. The outdoor play area was not inspected due to inclement weather. T he results of the inspection were reviewed with the provider during the exit interview. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-900-A
Description: Based on observation and provider interviews, the provider did not ensure that to assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise.

Evidence: Eight children were observed eating lunch that included bananas that were cut in thick, round slices. Three children were age 2 and two children were age 3. The bananas were prepared in a manner that poses a choking hazard to the children under age 4.

Plan of Correction: The provider will slice them in the future.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review and provider interviews, the provider did not ensure
to obtain a current Report of Tuberculosis Screening every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence: The provider will have the TB test taken.
