Comments:
An unannounced renewal inspection was conducted today from 9:40am to 12:00pm with the provider and three assistants. There were seven children in care, ranging from 14 months to three years of age, for a total of 17 points. Children were observed coloring, reading books, doing a puzzle, using the restroom and having diapers changed, washing hands for lunch, and eating lunch. Five children?s records, four staff records, and two household members? records were reviewed. The provider and all assistants have current certification in CPR and First Aid. The provider does not administer medication and must obtain certification in Medication Administration Training (MAT) to do so. Lunch today was chicken nuggets, apples, carrots, wheat bread, and milk. The first aid kit, flashlight, and weather band radio were observed and were in compliance. The emergency drill log, attendance, and menu were also reviewed. Violations:
Standard #: 22VAC40-111-830-D
Description: Based on review of documentation and interview with the provider, records of emergency evacuation and shelter-in-place drills were not maintained for one year. Evidence: The emergency drill log provided was for the current year, 2020. The Licensing Inspector requested the log for 2019 and the provider stated that he did not know he needed to save it and had thrown it away.

Plan of Correction: I believe I have some of the dates saved in my computer but I was not able to access them during the inspection. I will make sure to save the logs for one year moving forward.

Standard #: 22VAC40-111-180-A
Description: Based on review of four caregiver records, the provider did not ensure that all caregivers received a subsequent tuberculosis (TB) test or screening within two years from the date of the first test or screening. Evidence: The most recent TB test on record for Caregiver A was dated 2/13/17.

Plan of Correction: The caregiver will get an updated test as soon as possible, hopefully this weekend.

Standard #: 22VAC40-111-210-A
Description: Based on review of documentation and interview with the provider, the provider did not obtain 16 hours of annual training. Evidence: The training log for the provider documented five hours of annual training and the provider confirmed that this was accurate.

Plan of Correction: I will make sure I complete 16 hours of training per year moving forward.
