Comments:
27 children present with 8 classroom staff during this monitoring visit. The visit also included the investigation of a self reported incident that happened at the center. The children are playing in muddy puddles on the playground and looking for insects under the logs on the back playground. The children were also observed playing in their rooms, reading and playing games. Having lunch, today's lunch was mac and cheese, mixed veggies and oranges. Interaction between staff and children observed in both rooms and on the playground. Lots of supplies for the children to explore both inside and out. This visit was started at 10 and ended at 1:40. Standard #: 22VAC40-185-70-A
Description: The center failed to get subsequent background checks for 3 out of 5 staff files reviewed. Evidence: Based on the review of 5 staff records , 3 of the records have background checks that expired at the beginning of this year. Staff B, staff D and staff E are missing subsequent background checks.

Plan of Correction: We will start the process of getting the subsequent background checks done.

Standard #: 22VAC40-185-340-F
Description: The center failed to ensure that children under ten were in sight and sound supervision when a child, child A, was left out of sight and sound supervision on two dates. Evidence: Based on the review of the available facts, a child was left out of sight and sound supervision on October 22 and November 6th. The child a 3 year old boy went inside after his group returned from toad mountain , located at the back of the property to the tadpole yard which is right by the entrance to the rooms. On Oct. 22nd the child was found by a parent in the bathroom. The child was undressed and was found by a parent. The rest of the children were playing on the tadpole playground at the time. The staff went in to get the child and he stated to the staff that he had gotten wet and wanted to change his clothing. According to staff he was in the bathroom for about 10 minutes. On November 7th the child was counted during the transition from Toad mountain at 5:15 the children went to the tadpole room after playing in the tadpole yard for about 5 minutes. At 5:30 the child came up from the downstairs room by way of the door from the outside,with soap in his hair and on his hands and no shirt. He went to a teacher and when she asked where he had been he said he had been downstairs because he needed to change. Some parents stated that they had seen him but thought he was there with a parent. The child was downstairs alone for approximately 15 minutes without supervision. The staff failed to realize that the child was not part of the group.

Plan of Correction: We are changing the way we do evening transition, the children will go directly to their usual classroom and not be combined until numbers are lower. We will also make sure we remind staff to count the children. We are adding hours to a part time person and making sure there is a familiar face for the child. We are going to hire an afternoon teacher since the child has a hard time with transitions and unfamiliar staff.
