Comments:
An unannounced monitoring inspection was conducted on 11/12/2014 from 10:25am - 12:35pm. During the inspection there were 75 children ages two months to six years old in care with 16 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms including arts and crafts, writing, playing with blocks and story time. In addition, children were observed playing outside, eating lunch, and during rest period. Records were reviewed for five children and five staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice. lease complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure that all areas and equipment of the center, inside and out, shall be maintained in a safe and operable condition. Evidence 1. There were area rugs in both green classrooms, the yellow classroom and the orange classroom with fraying edges on multiple sides of the rugs. The strings where the carpet was fraying in some areas were in excess of 1 inch. In addition, there were corners on some of the rugs that were curled up and could present a tripping hazard. 2. The play structure located on the left side of the facility, directly outside of the Preschool rooms, and the play structure located in the back of the facility, directly outside of the Toddler rooms, both had areas where the paint is peeling and flaking. There is rust in the exposed areas where the paint is missing.

Plan of Correction: The fraying edges of the rugs will be trimmed and corners fixed so they are not a tripping hazard. A work order will be placed to have the play structures repaired (painted).
