Technical Assistance:
It was discussed that if it is known that a child may need emergency medication and the parent has not provided the medication, that staff must be made aware of exactly what the expectations are should the child show signs of needing the medication. Should a child no longer need emergency medication, this must be documented in the record and the information shared with the classroom staff.

Comments:
The inspector arrived at 1:10 PM and departed at 3:20 PM. Nap time and activities were observed. Sixty-four children were in care with 8 staff. A sampling of five children's records were reviewed. Medication administration was reviewed. The facility's plans of correction were obtained during the inspection. Please contact the inspector at (757) 491-3975 if there are any questions.

Standard #: 22VAC40-185-60-A
Description: Based upon review of children's records, the facility has not ensured that the record for each enrolled child contains the child's first date of attendance. Evidence: The records for children numbers 1, 2, 3 and 4 do not include the first dates of attendance.

Plan of Correction: Staff will enter the first date of attendance into each child's record.

Standard #: 22VAC40-185-270-A
Description: Based upon a tour, the facility has not ensured that all areas are maintained to be safe. Evidence: There is a room with a purple door along the main hallway directly across the hall from the boys' bathroom. During the inspection, the door to this room was unlocked. The area is being used as a storage area. There was an electrical cord laying on the floor. The cord could be a potential tripping or entanglement hazard. An electrical outlet approximately one foot above the floor has been pulled out of the wall exposing the electrical wires. Staff were unaware that the door was not locked.

Plan of Correction: Staff removed dangerous items from the room and will ensure that this room is kept locked at all times when children are in care. Staff will check all hallway doors to ensure they are locked at the beginning of each day and throughout the day,

Standard #: 22VAC40-185-280-B
Description: Based upon a tour, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: There is a room with a purple door along the main hallway directly across the hall from the boys' bathroom. During the inspection, the door to this room was unlocked. The area is being used as a storage area. There was an aerosol container of window sealant labeled "caution, flammable, keep out of reach of children" on the floor. An aerosol container of condenser coil cleaner, labeled "warning, keep out of reach of children" was on a shelf. On another shelf there was an aerosol container of Fresh and Clean odor neutralizer labeled "danger, flammable, keep out of reach of children". Staff were unaware that the door was not locked.

Plan of Correction: Staff removed dangerous items from the room and will ensure that this room is kept locked at all times when children are in care. Staff will check all hallway doors to ensure they are locked at the beginning of each day and throughout the day,

Standard #: 22VAC40-185-450-A
Description: Based upon observation, the facility has not ensured that during the designated rest period mats have linens consisting of a top cover and a bottom cover. Evidence: There were six children in staff 1's classroom napping on mats without top covers. The bottom covers on which the children were lying were too small to be folded over for use as a top cover also.

Plan of Correction: Staff will ensure that there are linens consisting of a top cover and a bottom cover for children resting on mats.
