- Technical Assistance:
Technical assistance was given regarding the clear documentation of required phone numbers for children's records.
An unannounced monitoring inspection was conducted on November 20,2014. At the time of entrance there were 6 children and 3 staff members present. The sample size consisted of 3 children's records and 3 staff records. Children were observed working on an art project, reading books and playing with cars. The inspection began at approximately 7:15 AM and ended at approximately 8:50 AM. There are no medications being administered this time. Areas of non-compliance are noted on the violation notice. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-140-A Description: Based on a record review and staff interviews, the licensee did not ensue each child had documentation in their record within one month of attendance of a physical examination by or under the direction of a physician. Evidence: During the inspection on November 20, 2014, the record for child #3 did not include documentation of a physical examination. Date of attendance September 3, 2014. Plan of Correction: The center supervisor will contact the parent to bring in all necessary documents to have a complete record on file.
Standard #: 22VAC40-185-60-A Description: Based on a record review and staff interviews, the licensee did not ensue each child had a record containing required information. Evidence: During the inspection on November 20, 2014, the record for child #2 did not include the 2nd emergency contact name, phone number or home address. Plan of Correction: The center supervisor will contact the parent to bring in all necessary documents to have a complete record on file.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not ensure hazardous chemicals are kept locked. Evidence: During the inspection on November 20, 2014, the Licensing Inspector observed the following; 1. In the 1st classroom stored on the bottom and middle shelves of an unlocked cabinet, there were 6 bottles of Bleach, 8 bottles of Pine Sol, 2 spray bottles of disinfectant cleaner, 1 container of disinfectant wipes, and 8 containers of hand sanitizer 2. In the 2nd classroom stored on the bottom shelf of an unlocked cabinet, there was 1 spray bottles of disinfectant cleaner, 1 spray bottle of bleach and water and 1 containers of hand sanitizer All items were labeled "keep out of reach of children" and at least one of the following "caution", "danger", and "flammable". Plan of Correction: The center supervisor will review with all staff that once they remove an item from the locked cabinet, they will immediately lock it back.
Standard #: 22VAC40-185-550-M Description: Based on a record review and interviews, the licensee did not ensure the center maintained a written record of children's serious and minor injuries in which entries are made the day of occurrence and the required information was documented. Evidence: During the inspection on November 20, 2014, 3 injury reports were reviewed and 2 of them did not include how the parent was notified of the injury and 1 did not include the time the parent was notified. Plan of Correction: The center supervisor will update the forms to make sure they have all the necessary information documented.
