Comments:
An unannounced monitoring inspection was conducted today, November 19, 2014 to determine the center's compliance with licensing standards. Some but not all standards were reviewed. The inspector arrived at 1:30 PM and concluded the inspection at 3:30 pm. Upon the inspectors arrival children were napping. Soft music played in the back ground. The three and four year old classrooms were combined for nap time. Later children were observed engaged in various activities. The three and four year old classroom had been working on an art project of making pop-cycle ornaments for the Festival of Trees event that the center is hosting. Children also had time on the playground where they played on the climbing equipment and on the swings. The two's and toddlers read stories and had snack before heading outside to the playground. Children provide their own meals. Children also practiced putting on their own jackets with the "flip over your head technique". Children were excited when they accomplished this move and were eager to help their classmates. Classrooms had been recently painted with bright colors and unused furniture had been removed giving the classrooms extra space. All rooms of the facility were inspected to include the front entrance play area, the gym, office space, bathrooms and four classrooms. Classrooms had age and stage appropriate materials and were decorated with children's art work and seasonal decorations. Staff interactions were positive and encouraged team work and following of classroom rules. The school age program is in attendance on Monday, Wednesday and Friday. On Tuesday and Thursday the school age program is held across the street at the YMCA main building. The playground s were inspected. The center's first aide kit and emergency supplies were inspected and found to be complete. The License, daily schedule, emergency evacuation maps, emergency numbers and various parent information was posted. The inspector interacted with staff and children during the inspection. Two staff (plus three follow up staff records) and four children's records were reviewed. Medication is being administered, five staff have current MAT certification. Multiple staff have current CPR and first aid training along with daily health observations. The last evacuation drill was conducted on 10/28/2014, One of the two required annual shelter-in-place drills was conducted on 03/25/2014. A second drill will need to be completed by the years end. Fire Inspection: 04/03/2014 Health Inspection: 03/13/2014 Today's ratio's observed : Toddler: 4 children with 1 staff. Two's: 5 children with 1 staff. Three's and four year old: 23 children with 3 staff Injury reports reviewed. The violations from the previous inspection on February 26, 2014 were checked for corrections. Standard #: 22VAC40-185-270-A
Description: Based on observations, the facility did not ensure all areas of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The small little tykes climber on the toddler/ two's playground has a rope net that is intended for climbing up to the platform. The netting has come loose from the bottom of the equipment and has created a loop that can entangle a child or cause a child to trip.

Plan of Correction: Per the director: I will contact our maintenance to repair the netting and reattach it to the equipment.

Standard #: 22VAC40-185-290-3
Description: Based on observations, the facility did not ensure all electrical outlets had protective covers installed. Evidence: Two outlets in the multipurpose/ school age classroom did not have protective covers. The classroom is used for both school age children and preschool children.

Plan of Correction: Per the director: I will install the protective covers right away.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications, the facility did not ensure parents were notified, when an authorization for medication expired, that the medication needs to picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: The medication authorization for child #1, expired on 06/01/2014. The medication had not been returned to the parent nor had it been disposed of, exceeding the 14 day allowance period.

Plan of Correction: Per the director: We will contact the parent today and give her the option to renew the authorization or take the medication home.
