Comments:
Today's visit was a routine monitoring inspection. The inspection began at 9:00am and concluded at 1:45pm. There were thirty-three children present with five direct care staff and the Program Administrator was also present. During the inspection both of the playgrounds were viewed, all operating classrooms were observed, the school age room was not in use, lunch time was observed, two medications were present and authorization was reviewed, nap time was observed, three out of thirteen staff records were reviewed, four out of ninety-seven children's records were reviewed, and some policies and procedures were reviewed. Risk assessment was discussed at the conclusion of today's inspection and is located on the Supplemental page of today's inspection. Violations:
Standard #: 22VAC40-185-380-A
Description: Based on the inspector's observations, the center failed to ensure the posted schedule in two classrooms did not include all the elements of the center's daily schedule. Evidence: The inspector reviewed the schedules in all five classrooms. The center serves a morning snack and morning snack is not indicated on the schedule for the preschool classroom and the classroom with three year old children.

Plan of Correction: The staff will adjust their schedules to include the morning snack and will be posted by the end of the program day.

Standard #: 22VAC40-185-520-B
Description: Based on the inspector's review of the form used to authorize the use of sunscreen, the center failed to ensure the permission form for the use of sunscreen includes an area to note any known adverse reaction. Evidence: The inspector observed sunscreens present in each classroom. The permission form used to authorize the use of sunscreen does not include an area to indicate any known adverse reaction.

Plan of Correction: The office assistant will add this to the permission form and ask parent to re-authorize the use of sunscreen.

Description: Based on the inspector's staff interview, the center does not have a log to record the application of diaper creams/ointments with required information. Evidence: The inspector observed the two year old classroom and observed three containers of diaper cream/ointments. The inspector asked the staff member for the log that is used to record the application of diaper/creams ointments and the staff member stated she was not aware of one. A log with child's name, date of use, frequency of application and any adverse reaction is required for the application of diaper creams/ointments. Based on the inspector's review of the form used to authorize the use of diaper creams/ointments, the center failed to ensure the permission form for the use of diaper creams/ointments includes an area to note any known adverse reaction. Diaper creams/ointments were present in the two year old classroom. Evidence: The inspector observed sunscreens present in each classroom. The permission form used to authorize the use of diaper creams/ointments does not include an area to indicate any known adverse reaction.

Plan of Correction: A log will be in place the end of the program to document the application of diaper creams and permission forms will be revised to include adverse reactions if any.

Description: Based on the inspector's review of the form used to authorize the use of insect repellent, the center failed to ensure The permission form for the use of insect repellent does not include and area to note any known adverse reaction. Evidence: The inspector observed insect repellents. The permission form used to authorize the use of sunscreen does not include an area to indicate any known adverse reaction.

Plan of Correction: The permission forms will be revised to include any known adverse reactions.

Standard #: 22VAC40-185-560-F
Description: Based on the inspector's observations during morning snack time, the center failed to ensure the morning snack included a second component as required by USDA. Evidence: The inspector observed morning snack and goldfish crackers and water were served. The morning snack consisted of one component only , as water does not count as a second component.

Plan of Correction: A second component will be added to the menu by the next program day.
