The licensing inspector conducted an unannounced monitoring inspection Thursday, December 11, 2014 from 2:48pm to 5:30pm. The director accompanied the inspector on the visit. The center's regional director was on site training another center director. The director-in-training also accompanied the inspector on the visit as well. The children were observed in a variety of activities. The infants were observed having floor time and being changed. The toddlers and twos were observed eating snack, which consisted of applesauce and juice. Both the preschool and pre-k classes were finishing their snack and preparing to move on to the next activity. During the inspector's visit the school age children arrived to the center from school. They were observed playing card games and in the dramatic play learning center area. Some were observed doing their homework. A formal observation was completed with the infant class and the two year old class. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today. The facilities mini buses were not inspected due to them being used to transport the children from school to the center. Five children?s records and two employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/17/2014 Shelter-in-place drills: 2/21/2014, 4/25/2014, 10/20/2014 Last fire inspection: 2/4/2014 Last health inspection: 7/24/2014 Total enrollment: 73 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?4:1 Twos ? 6:1 Threes ?9:1 Pre-K ?20:2 School Age ?16:1 The violations from the previous inspection in April of 2014 were checked for corrections. There were five repeat violations found during today?s inspection. If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records on 04/30/2014, the center did not ensure that two children's records contained documentation of a physical examination before, or within on month of the child's first day of attendance. EVIDENCE: 1. The record of Child # 3, whose first day of attendance was 06/09/2014, does not contain documentation of a physical exam. 2. The record of Child # 5, whose first day of attendance was 04/23/2013, does not contain documentation of a physical exam. Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 12/11/2014, the center did not ensure that one of two staff records contained documentation of a negative tuberculosis (TB) screening no later than 21 days after employment. EVIDENCE: The earliest TB results documented in the record of Staff #1, whose hire date is 10/21/2014, is dated 12/10/2014. EVIDENCE: The record of Child # 4 did not contain documentation that center staff had viewed proof of the child's identity and age. Standard #: 22VAC40-185-280-B Description: Based on observation on 12/11/2014, the center did not ensure that hazardous substances such as cleaning materials, shall be kept in a locked place that prevents access by children. EVIDENCE: The inspector observed a spray bottle labeled bleach and water on top of the diaper shelf in the two's room. The spray bottle was out of the reach of children. Standard #: 22VAC40-185-330-B Description: Based on observation on 12/11/2014, the center did not ensure that the depth of resilient surfacing under playground equipment complied with minimum safety standards. EVIDENCE: 1. On the school age playground, there was less than 6" of pea gravel under the swing set equipment. 2. On the preschool playground, there was less than 6" of pea gravel under the slide equipment. All playground equipment with climbing apparatus' shall have resilient surfacing to create a fall zone free of hazardous obstacles. Plan of Correction: Not available online.
