Technical Assistance:
On the playground, the licensing inspector observed rusted S-hooks on the swings that were open more than the thickness of a penny. The resilient surfacing (mulch) was inadequate in some areas of the fall zones (4"-5") under and around the climbing wall and monkey bars. There was a large, rusted nail protruding from the wooden border surrounding the playground to the left of the center.

Comments:
An unannounced monitoring inspection was conducted on January 30, 2015 during the before care portion of the school age program. At arrival to the center, there were three children present with two staff members. There are no medications present or being administered by the program staff. The licensing inspector observed the children coloring, playing with cars and during an evacuation drill. Three children's records were reviewed.

Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the center failed to provide an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on January 30, 2015, the annual fire inspection report for the center had expired and was dated 10/31/13.

Plan of Correction: The Area Manager is trying to take care of getting an inspection completed.

Standard #: 22VAC40-185-550-M
Description: Based on review, in two of five injury reports reviewed, the staff members did not ensure that all required information was documented. Evidence: During the inspection conducted on January 30, 2015, injury reports dated 10/27/14 and 12/04/14 did not include documentation of any future action to prevent recurrence of the injury.

Plan of Correction: We will work extra hard to correct future incidents with the correct information on the forms.
