Technical Assistance:
Technical assistance today included record keeping: school-agers' physicals and shot records; documentation of verification of identification; previous child care; incident report forms; written agreements; other programs concurrently attending; documentation of parent conferences/assessments.; documentation of an injury log with the purpose of pinpointing patterns of concern. We also discussed access to the kitchen. Consultation today included concern regarding an over-the-counter medication authorization from a physician that exceeded the manufacturer's recommendation.

Comments:
An unannounced renewal inspection took place today from 10:45 - 2 pm. There were 55 children present with 7 teaching staff. I observed children busy with a variety of activities: outdoor play; painting with Q-tips; listening to a story; using droppers to create watercolor glitterpaint designs; involved in role-playing; building a fort of small rocks for plastic people figures; and motoring with pretend cars. A visiting music/movement teacher had the children pretend ice skating in a circle. The center looks very inviting with fresh paint. Violations:
Standard #: 22VAC40-185-50-A
Description: During today's inspection, certain children's information was observed to be posted in two classrooms, not protecting confidentiality as required. Evidence: Children's allergy and other information were posted without documentation of parental consent.

Plan of Correction: The allergy list will be covered for privacy today.

Standard #: 22VAC40-185-60-A
Description: A review of a sampling of children's records found some missing information. Evidence: Of four children's files reviewed today, there was no work phone number for Child #1 and no physical exam and shot record for Child # 3.

Plan of Correction: We will get the information from the parent and have all school-agers get the required physical exams and immunization records.

Standard #: 22VAC40-185-270-A
Description: During today's inspection, some outdoor areas were observed to not be maintained in a safe condition. Evidence: 1. A soft-sided swing seat was observed to be torn or trimmed of some of the seat. 2. Areas of splintering were seen on wooden play structures and on the outside barn wall accessible to children. 3. A piece of brick was missing from the front sidewalk entrance, posing a tripping hazard. 4. A long, unattached wooden board was supported by across two tree branches approximately four feet from the ground, posing a falling hazard. 5. The homemade xylophone made of boards was observed today strung between two trees on a string just above child height but the gaps between the boards posed a hazard. 6. Outdoor toys: A toy truck was observed to be rusted; a piece of plastic was broken on a riding toy; and a metal bike was broken. 7. Some pieces of mulch were observed today to be over six inches in one area of the younger children's playground. 8. The metal slide in the younger children's playground had no side safety rail for holding as children slide down; there was a small gap at the top and the chain link climbing area was rusted. A larger metal slide in the older children's area was badly pitted. 9. The back of two bolts were exposed and accessible on the metal climbing bars.

Plan of Correction: The swing will be off-limits until it is replaced. Wooden areas will be sanded this weekend. The brick will be replaced this weekend. The board was removed. The person who constructed the xylophone will be contacted to add boards and strengthen the string. The toys will be removed or repaired. The metal slide will be made off limits. The bolts will be covered. When the mulch was ordered, the first delivery had larger pieces but the rest was double-shredded. We will remove the larger pieces.

Standard #: 22VAC40-185-270-B
Description: During today's inspection, a radiator supplying heat in one classroom was observed to be missing appropriate barriers to prevent possible injury. Evidence: In one classroom, all radiators but one had a protective barrier built in to prevent accessibility. The one built-in radiator without the barrier was observed today to be hot to the touch and within reach of children.

Plan of Correction: Staff blocked off the area today. A cover will be ordered or one will be built.

Standard #: 22VAC40-185-510-E
Description: A review of children's medications today found that not all were labeled as required. Evidence: Over the counter allergy medication was labeled with names but not dosages as required.

Plan of Correction: This will be put on the bottle or the box.

Standard #: 22VAC40-185-510-G
Description: A review of medication authorizations today found that one parent authorization had expired. Evidence: The parent authorization for medication for Child # 2 expired 12/14.

Plan of Correction: I will call the parent today.

Standard #: 22VAC40-185-550-M
Description: A review of a sample of a children's incident/injury report today found some information missing. Evidence: For the most recent report, there was no documentation of the time that the parent was notified and no future action to take to prevent recurrence of the injury as required.

Plan of Correction: I will go over this tomorrow with staff and at the Thursday staff meeting.
