Technical Assistance:
We discussed the requirements for the age and identity law. The risk assessment for a violation was provided. We talked about the additional equipment you plan to add to the back playground. Thank you for making sure the detaching net on the school house is removed prior to use. The Junior-K children are no longer using the front playground areas. As a reminder, sixteen hours of annual staff development are required for staff who work with the children.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:00 AM to 1:25 PM. Today, there were eleven Junior-K children with two teachers. Nine children remained for lunch and the afternoon hours. I viewed program activities, daily routines, staffing, supervision, interactions with children, snack and lunch, posted information, three records for children, medication record for one child, two records for staff, staff training, emergency supplies and drill records, indoor and outdoor areas. The children have enjoyed learning about President Lincoln, and winter related themes. Today, they had library time and music. Due to the weather, indoor movement was provided using videos that guided children in dance and yoga. A variety of materials were accessible for children to use if they were finished with the movement activities. Teachers were observed providing positive guidance and responding to individual needs for assistance. Hand washing reminders were provided after snack/lunch.

Description: Based on a review of three records for children, and an interview with administrative staff, the proof of age and identity for child 1 was not viewed within seven business days of initial attendance. Evidence: The first day of attendance was November 21, 2014. Administrative staff indicated that the child's registration forms and health records were obtained at the start of the school year and attendance was delayed without notice that a birth record was not presented to school staff.

Plan of Correction: The teacher immediately sent an e-mail to a parent and asked for a birth record to see while the parent attends a school event on Friday. The proof of identity will be viewed and documented for the child's record. In the future, the teacher will make sure the birth records are viewed at the time of enrollment or within seven business days of initial attendance. If the requested birth records are not provided as required, the designated school staff will report the lack of proof of identity to the appropriate law enforcement agency.
