Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Staff of the Lovettsville Montessori School located in Lovettsville, Virginia. At the start of this unannounced renewal inspection there were a total of 16 children with 2 staff; good ratio for the one large group. A total of 16 children?s files and 2 staff files were reviewed. The children were engaged in the following: Valentine's party, story time, song, dance, games, learning activities, hand washing, bathroom breaks, lunch, clean up, pick up, and other organized activities. Standard #: 22VAC40-185-130-A Description: Based on review, 2 of 16 children's files reviewed indicated that the center center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child can attend the center. Evidence - 1. On the date of inspection (02/12/2015), immunization documentation was not available for: Child #14, and Child #15. 2. The start dates were listed as: 02/02/2015 for Child #14; and 02/09/2015 for Child #15. Plan of Correction: Immunization documentation will be obtained for both children.
Standard #: 22VAC40-185-240-D-5 Description: Based on review and interview, there is not at least one staff member on duty who has obtained within the last three years instruction in performing the Daily Health Observation (DHO) of children. Evidence - 1. On the date of inspection (02/12/2015), there was not a staff person on site with a current DHO training certification. 2. The most recent documentation observed on site was dated - 11/16/2011. 3. The Director stated (during an interview conducted on the date of inspection) that she thought that the certification was only needed to be obtained once. Plan of Correction: Additional training will be obtained.
