Technical Assistance:
The licensing inspector recommended to the program director that she should contact Child Protective Services regarding an incident that presented itself during staff record review.

Comments:
Today an unannounced renewal study was conducted. The inspection began at 11:15am and concluded at 1:40pm. Staff records, chldren's records, medication, first aid, certifications, and fire inspections were reviewed. Children were observed listening to stories, washing hands, preparing for nap, and sharing projects with their peers. Staff were engaged with the children and assisting with hand washing. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on physical plant, site failed to maintain a safe environment. Evidence: The door leading downstairs to the Pre-K area was not able to be completely closed making the safety knob ineffective.

Plan of Correction: We will contact maintenance to fix the door.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, a non-prescription medication was at the facility longer than 10 work days without a physician's authorization. Evidence: An over the counter medication dated 1/27/14 was in the medication box without the proper physician's signature.

Plan of Correction: We will obtain the correct form from the parents.

Standard #: 22VAC40-185-550-M
Description: Based on observation, written record of children's injuries is incomplete. Evidence: 2 of 5 injury reports did not have when the parent was notified and who notified the parent.

Plan of Correction: We will fill the form out directly after informing the parent of an injury.
