Comments:
This was an initial inspection for licensure. During this inspection standards related to care of children, programming and children's records could not be determined as there were not any children in care. All areas of standards will be determined during the first unannounced inspection. Going forward, all inspections will be unannounced. The first unannounced inspection will be conducted within 30 - 60 days of receipt of the paper license. The recommendation will be for a conditional, six month license with a recommended capacity of 35 children. Stipulations to appear on the license will include not more than 2 infants in care at anyone time. No children under 2.5 years of age in care on the second floor. The outside play ground will be limited to no more than children at anyone time.

Standard #: 22VAC40-185-70-A
Description: Based on observation and interview the center failed to ensure that each staff record contains all required information. Evidence: 1. The record for staff 2 does not include a date of hire or a job title. The staff was present working in the center during the inspection. 2. The record for staff 3 does not include a date of hire or a job title. The staff was present working in the center during the inspection. 3. Center staff and management persons present during the inspection confirm that the information was not present in the staff records.

Plan of Correction: Center management will ensure each staff record contains all required information including but not limited to date of hire and job title.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that equipment is maintained in safe condition. Evidence: 1. There is a set of wooden cubbies in the toddler/two room that is not secured to a wall. Therefore, the cubbies pose a toppling hazard. 2. Staff present confirm the cubbies are not secured to the wall. 3. There is a whole in the wall in the infant room. The whole measures 1/4 inch in diameter. The whole is located within reach of one of the three cribs in the room. 4. Staff present confirmed there is a whole in the wall in the infant room.

Plan of Correction: Going forward all cubbies installed in the center will be secured in such a way that they do not pose a toppling hazard. The existing cubbies will be secured as soon as possible. The whole in the wall in the infant room will be filled as soon as possible.

Standard #: 22VAC40-185-510-J
Description: Based on observation and interview the center failed to ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Staff 1, when asked gave the inspector a first kit. The kit was located on a shelf, approximately 6 1/2 feet above the floor. 2. Shelf is an unlocked location. 3. There were six single dose packages of pain reliever/ fever reducing medication. 4. There were four single dosage packets of medicated ointment and creams. 5. There is an identical first aid kit in the toddler/2's classroom that is stored on top of a file cabinet. An unlocked location. 5. Center staff and management persons present during the inspection confirm that there was medication in the first aid kit and that the kit was stored in an unlocked location.

Plan of Correction: Corrected during inspection. Center management will ensure that all medication is stored in a locked location.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview the center failed to ensure that there is one working, battery-operated radio in each building. Evidence: 1. When asked center management provided a radio. However, the radio did not have any batteries. 2. Center management present during the inspection confirm that the radio did have batteries.

Plan of Correction: Center management will make sure that there are batteries available for the radio at all times.
