Comments:
An unannounced monitoring inspection was conducted on March 3, 2015. At the time of entrance there were 22 children and 2 staff members present. The children were observed eating snack, playing board games and working on homework. The playground was not inspected due to inclement weather. Three medications were reviewed. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The inspection began at approximately 3:15 pm and ended at approximately 4:50 pm.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review, the licensee did not ensure there was a record at the center containing all required information. Evidence: During the inspection conducted on March 3, 2015, the records for child #2 and child #3 did not include the employment phone number for one of the parents listed. The record for child #1 (first date of attendance 11-14-14) and child #3 (first date of attendance 1-15-15) did not include a copy of a physical. Physicals are required within one month of the child's attendance.

Plan of Correction: The center supervisor will contact the parent to update their file to have all the information in their record complete.

Standard #: 22VAC40-185-550-D
Description: Based on a documentation review, the licensee did not ensure the center implemented a monthly practice evacuation drill. Evidence: During the inspection conducted on March 3, 2015, there was not documentation indicating a drill had been conducted in February 2015.

Plan of Correction: The center supervisor will ensure all drills are documented for review.

Standard #: 22VAC40-185-550-M
Description: Based on a documentation review, the licensee did not ensure there was a written record of children's serious and minor injuries in which entries are made the day of occurrence and included all required information. Evidence: During the inspection conducted on March 3, 2015, thirteen injury reports were reviewed and four did not include how the parent was notified of the injury and two did not include the time the parent was notified of the injury.

Plan of Correction: The center supervisor will update their forms to ensure they have all the necessary information documented.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the licensee did not ensure a menu listing foods to be served for meals and snacks during the current one-week period shall was placed in a conspicuous place for parents to review. Evidence: During the inspection conducted on March 3, 2015, the menu posted was dated for the month of February 2015.

Plan of Correction: The center supervisor will ensure the current menu is posted for parents to review.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the licensee did not ensure the food container was sealed and clearly dated and labeled in a way that identifies the owner. Evidence: During the inspection conducted on March 3, 2015, five children brought food from home and four did not include the date and a label that identifies the owner.

Plan of Correction: The center supervisor will ensure all snacks brought from home are labeled.
