Comments:
An unannounced monitoring inspection of the facility and children's records was conducted from 9:51am ? 11:35am. There were 14 children in care with 2 staff members. Interviews were conducted with children and staff present. Four children?s records were reviewed. Staff records are stored at a central location. However, emergency contact information for each staff employed at this location was reviewed today. Children were observed eating lunch, transitions, hand washing, rest room routines and circle time. Any repeat violations and or areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that equipment is maintained in safe condition. Evidence: 1. There is a four foot tall set of wooden, coat cubbies that is not secured to the wall and poses a toppling hazard. The cubbies are located between the entrance to the classroom and the girls bathroom. 2. Children were observed standing next to the cubbies while waiting to enter the bathroom with and without a staff member close by. 3. Adults were observed entering and children were observed leaving the classroom through the door next to the unsecured cubbies. 4. Staff 1 and 2 confirmed that the cubbies were not secured.

Plan of Correction: The cubbies had been secured at one time. Center staff will request that the cubbies be secured again. Center staff will periodically check cubbies to ensure that they are secured. Center staff will ensure that children do not lean on or play near the cubbies while they are not secured.

Standard #: 22VAC40-185-270-E
Description: Based on observation and interview the center failed to ensure that safe drinking water shall be accessible at all times. Evidence: 1. Child 1 requested a drink a drink of water. Staff 1 instructed the child to go get a drink. Child 2 got up, however the child did not say anything. Staff 1 stated "uh ah, child 2 someone does something and you want to follow behind. We not doing that." Child 2 sat down. 2. Staff 1 confirmed that child 2 was not permitted to get a drink of water.

Plan of Correction: Agency management will remind and retrain all staff regarding the requirement that water shall be accessible at all times.

Standard #: 22VAC40-185-550-F
Description: Based on observation and interview the center failed to ensure that 911 and poison control shall be posted in a visible place at each telephone. Evidence: 1. Emergency numbers are not posted or visible at the wall mounted phone in the classroom. There is a phone on the wall next to the parent entrance of the child care room. 2. Staff 1 and 2 confirmed that the emergency numbers were not posted.

Plan of Correction: Center staff will ensure that required emergency numbers are posted at each telephone in the center.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview the center failed to ensure that the posted menu was for the current one-week period and dated. Evidence: 1. The inspection took place on 3/2/15 and began at 9:51am. 2. The posted menu was labeled for the month of February. 3.. Staff 1 and staff 2 confirmed that the menu posted was for the month of February.

Plan of Correction: Center management will ensure that the correct, current menu is posted in the facility.
