Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 2 pm. Today, I met with the Staff of Children of America located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 112 children with 18 staff; good ratios in all 11 rooms. A total of 3 children?s files and 2 new staff files were reviewed. The children were engaged in the following: art, story time, reading, games, diapering changing, bathroom breaks, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B Description: Based on review, 1 of 3 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for a child under the age of two years. Evidence - 1. On the date of inspection (03/11/2015), the most recent immunization documentation on file for Child #3 was dated - 09/04/2014. 2. It should also be noted that this documentation was neither stamped nor signed by the child's doctor's office. The forms were only initialed with no further indication by whom. Plan of Correction: An updated immunization page will be obtained. The parents will be instructed to obtain documentation either stamped or signed by their doctor's office.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all electrical outlets had protective covers. Evidence - 1. On the date of inspection (03/11/2015), within the "City Place" room, an outlet was observed missing its cover. 2. The Center Director confirmed this observation on the date of inspection. Plan of Correction: A cover will be placed in this outlet.
Standard #: 22VAC40-185-510-A Description: Based on review, 1 of 3 medications reviewed indicated that a parents signed authorization was not available. Evidence - 1. On the date of inspection (03/11/2015), the parent signature was missing from the medication authorization form for medication prescribed for Child #1. 2. This finding was confirmed on the date of inspection by the Center Director. Plan of Correction: The parent will be contacted and the authorizing signature obtained.
Standard #: 22VAC40-185-570-E Description: Based on observation, a refrigerated bottle of prepared infant formula was not dated and labeled with the child's name. Evidence - 1. On the date of inspection (03/11/2015), a bottle located in the refrigerator within the Infant B room was without the child's name and current date. 2. The bottle was observed in a tray labeled with the child's name, yet no name (or date) was placed on the actual bottle. Plan of Correction: Corrected on the date of inspection. The name and current date was placed on the bottle.
