An unannounced monitoring inspection was conducted today. There were 34 children in care, with a total of 3 staff members assigned to their direct supervision. This inspection took place on a public school closure day. The children were observed playing outside and eating lunch brought from home. Earlier in the day, the children had participated in a group cooking project. This inspection was completed between approximately 11:40 am and 12:30 pm. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that four records were not complete. Evidence: 1. Record #1 does not include the documented date of enrollment. 2. Record #2 does not include: an annual review of the child's contact information (this record was most recently reviewed by the parent on 9/3/13); information in the "authorized pick ups" section on the child's registration form. 3. Record #3 does not include: the father's work phone number; the physician's phone number; a physical exam report; a shot record. 4. Record #5 does not include: the mother's employment contact information; the documented date of enrollment. Plan of Correction: The parents will be asked for the missing information. The records will be reviewed and updated.
Standard #: 22VAC40-185-550-D Description: Based on a review of emergency drill documentation, it was determined that there is no record of a fire evacuation drill each month. Evidence: There is no documentation to indicate that the center practiced a fire evacuation drill during the months of January and February, 2015. Plan of Correction: We did the fire drill each of those months but forgot to write them down. I will remember to document the fire drill every month.
