Discussed with the Director: Documentation of Lead Teacher Qualifications; supplies for School Age classroom and 2 1/2 - 3's; Update Injury Prevention Plan based on a review of the Accidents.
Unannounced renewal inspection was conducted beginning at 9:00 and ending at 1:00 . There were 96 children present, ages 2 months through 5 years of age. All classrooms were in appropriate ratios. Children were observed going group and individual activities, including puzzles, art, circle time and infant feeding. Good transition activities were observed in the Pre K classroom when the group moved from Circle to Table work. The Licensing Inspector enjoyed chatting with the children about their visit from the leprechaun and all that he did. Reviewed 10 children's files - see Violation Notice. Reviewed 5 staff files - discussed with the Director Emergency Escape Drills and Shelter in Place Drills were up to date as are Health and Fire Inspections. Reviewed Accident /Injury Reports - complete. Reviewed the two medications - complete Licensing Inspector Linda Reynolds 540-272-2663
Standard #: 22VAC40-185-50-A Description: Based on classroom observation, it was determined that there was confidential information accessible to all in one classroom. Evidence: The Allergy List was on a classroom board in the School Age Classroom. It was not covered or kept where anyone could read manes and information. Plan of Correction: Management staff will assure that the Allery List is either covered or kept inside a cabinet
Standard #: 22VAC40-185-60-A Description: Based on a review of 10 children's files, it was determined that 6 of 10 files had incomplete information. Evidence: Child #1 had no custody paper; Child #3 had no physical on file - enrolled 9/14; Child #4 had no doctor's phone number; Child 6 had no doctor's phone number and did not have a full address for 1 emergency contact; Child #8 did not have complete addresses for emergency contacts; Child #9 did not have a physician's name and phone number. Plan of Correction: The Director and the Assistant Director will review all children's files for complete information.
