I strongly suggest that certain person's collect information about children's allergies and then distribute that information to the teachers and the cook. All paperwork on file should consistently identify the current allergy or intolerance and when changes are made then it should be documented in writing from the parent/guardian/physician. If you are going to post allergies in the classrooms/kitchen you should date the allergy list indicating when it was last updated and make sure current allergy information is immediately updated on the posted list.
A complaint was received involving care, protection and guidance. Interviews, observation and a review of documentation resulted in a valid complaint. Please submit your plan of correction to the Division of Licensing by 3/16/15.
Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on interviews, observation and a review of record keeping, it was determined that the center is not always ensuring the care and protection of children as one child was exposed to an allergen while at the center. Evidence: Child A enrolled on 9/30/14 at the center was informed that she was lactose intolerant and an alternate milk was provided. The child's file listed her as having a milk allergy. At the beginning of November 2014 the center ran out of the alternate milk for child A; therefore during lunch, the center/staff allowed the child to consume cows milk on average once a week. Consequently, in February 2015, the parent traced the digestive issues the child was having to the milk exposure at the center. Additionally, the child's allergy was not on the allergy list which is kept posted in each classroom and the kitchen until it was brought to the center's attention in February 2015. Plan of Correction: All the classrooms have been given an updated allergy list. The cook has been given an updated list as well. In addition to that, the teachers have been trained not to take verbal changes regarding a child allergy. The teachers will request a written changes or adjustment to any changes and also to direct the parent to the front desk to come talk to management. In addition to this we have started highlighting each child name on the attendance sheet.
