Comments:
An unannounced renewal inspection was conducted today by the licensing inspector. The center director was present and assisted during the inspection. All required information was posted, vehicles and the playground were inspected and a sample of staff and children's records were reviewed. There were no children present during inspection of the school age building. The outdoor pool gates were locked. staff/child ratios observed: 4's 1:7 Children were playing in various interest areas and had done a Spring kite art project. pre K 1:8 Some of the children were playing with blocks and trains, doing a matching game, and transitioning to a music/movement activity. 3's 1:9 The group was practicing writing the alphabet. 3's 2:14 They were making a handprint rainbow, playing in the housekeeping area and creating a colorful bird using feathers. 2's 1:7 The staff person was reading a story to the children. 2's 1:7 The staff person was reading a story to the children. 2's & 3's 1:4 Children were listening to Frozen and transitioning to lunch. toddlers 3:11 The children were having lunch. infants 2:7 Children were sleeping, playing on the floor, being held, fed and sitting in infant seats on the floor. today's menu: breakfast (optional) milk or juice, turkey sausage, cereal morning snack - fruit and milk afternoon snack - turkey & cheese wrap lunch - chicken strips, broccoli or snap beans, fruited jello, milk liability insurance expiration 6/15/15 health inspection 12/8/14 fire inspection 10/15/14 first aid/CPR training 14 staff health observation training 5 staff MAT (medication training) 5 staff approximate time on site: 10:40 am - 2 pm

Standard #: 22VAC40-185-510-C
Description: Based on inspection of medication, the facility did not ensure a medication was dated within the expiration date. Evidence: An Epi pen for a child in care had an expiration date of January 2015. The authorization form had been signed January 2015.

Plan of Correction: We will request another Epi Pen with a valid expiration date from the parent today.
