Comments:
Conducted an unannounced monitoring study with the Director, 14 staff, and 48 children. The ages of the children range from nine weeks-old to five years-old. The children were taking Spring pictures, playing on the playground, having free play in the classrooms, eating lunch, and napping during the inspections. Two new staff records and five children's records were reviewed. Start time: 10:00am End time: 1:00pm

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on a direct observation of of five classrooms, there were some items in disrepair. Evidence: In classroom 1 and classroom 7, there was one carpet in each classroom that was torn and tattered at the edges, posing as a tripping hazard. In classroom 2, there was a musical instrument, tambourine, that was torn and tattered. In classrooms 3, 4, and 5, the toilets in the bathrooms were soiled, stained, and cracked at the base. In classroom 5, the middle toilet's tank top was tilted. In classroom 7, there was one crib sheet that was stained. In classroom 5 , there were three chair mats torn and tattered at the corners.

Plan of Correction: Tattered carpet was discarded from room 1 and 7 Tambourine was discarded from room 2 Maintenance has come to re-caulk the toilets in room 3, 4, and 5. Maintenance will replace toilet tank top from room 5. Cleaning checklists will be done to ensure that areas are clean and free of stains.

Description: Based on a direct observation, there was one can of disinfectant spray sitting on a shelf. Evidence: In classroom 1, there was a can of disinfectant that was on the shelf and was not locked.

Plan of Correction: Disinfectant spray was locked in cabinet immediately.

Standard #: 22VAC40-185-570-E
Description: Based on a direct observation, there was one bottle missing information. Evidence: In classroom 7, there was one bottle that did not have a complete date labeled.

Plan of Correction: Staff will be retrained to ensure that each bottle entering the refrigerator is labeled with the first name, last name and days date.
