Technical Assistance:
A supplemental page was provided. Discussion took place regarding the change of class times for the different age groups for the 2015-16 school year.

Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted April 2, 2015. The on-site visit was from 11:00 a.m. to 2:10 p.m. On this date, there were seven (7) children present in the two-year-old morning class and seven (7) children present in the four-year-old afternoon class. A total of two (2) staff members were present during each class and four college students were present throughout the day. The total enrollment for all three classes was twenty-seven (27) children. The exception to compliance was noted on the violation notice and the risk rating for the violation was discussed. There were three (3) staff members' records reviewed and four (4) children's records reviewed. Three (3) staff members and four (4) children were interviewed. At the time of the visit, there was no medication being administered. Afternoon snack was observed to meet USDA guidelines and consisted of hard-boiled turkey eggs and a choice of milk or juice. Choice time, outdoor play, Easter egg hunt, dismissal procedures, arrival procedures, opening circle time, calendar time, story time, singing, small motor activities, woodworking, the making of Easter baskets, dying Easter eggs, snack, hand washing, and bathroom procedures were observed during the visit. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff records and verification from the staff member, it was determined that one staff member had not resubmitted the results of a follow-up tuberculosis (TB) screening within the last two years. Evidence: Three staff members' records were reviewed during the inspection on 04/02/2015. 1. Staff Member B had not obtained a follow-up TB screening or test with the last two years. 2. Staff Member B confirmed that she had not realized she was due for a follow-up screening. 3. The date of her last TB test was 02/25/2013.

Plan of Correction: The Staff Member will obtain a TB screening or test as soon as possible and the director will let the licensing office know as soon as she has obtained the results of the test.
