Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 3 pm through 5 pm. Today, I met with the Staff of the Ashburn Village Sports Pavilion located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 26 children with 5 staff; good ratios in both groups. A total of 3 children?s files and 3 staff files were reviewed. The children were engaged in the following: games, free play, leggos, swimming, snack (apple juice, pudding, and graham snacks), and other organized activities. Standard #: 22VAC40-191-60-D-1 Description: Based on review and interview, 1 of 3 staff files reviewed indicated that a requested report was not sent within seven calendar days of their start date. Evidence - 1. On the date of inspection (03/30/2015), the start date for Staff #2 was determined to be 12/03/2015. The completed Central Registry Release of Information form (CPS) for Staff #2 was notarized on 01/14/2015 and was stamped received on 03/13/2015 by the central registry. This request was returned for having a check being older than 90 days (check dated - 12/31/2015). 2. A representative from the Department of Social Services, Office of Background Investigations - Search Unit stated (during an interview conducted on the date of inspection) that the unit was in error and would correct the problem. Instructions were given to obtain a finding. Plan of Correction: The request form with a new check will be sent in order to obtain a finding.
