22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted in the afternoon to observe the children's activities during school-age "fun club". The county public schools were closed for spring break. Upon arrival to the program, this LI learned the children were off site participating in an outing at a local park. The LI verified 57 children were in attendance on inspection date, April 2, 2015. The LI observed the children as they played and participated in an Easter egg hunt at the park. Upon arrival back to the Center, the children were served an afternoon snack. During the late afternoon of the program day, the children played games outdoors on the center's grounds. A sample of records was reviewed, 5 children's and 1 staff member. Description: Based on the LI's observations, the center's license was not posted in a place conspicuous to the public. Evidence: The LI did not see the license posted so an inquiry was made to the SACC Coordinator. The Center's current license could not be located and reportedly had been misplaced or taken off of the information board.

Plan of Correction: The SACC Coordinator will request a duplicate license. The license will be posted.

Description: Based on a sample review of children's records, one of five records did not contain complete information. Evidence: The LI reviewed five children's records, the record of child #2 did not have a complete address listed for one of the emergency contacts.

Plan of Correction: The Child Care Coordinator will review children's records to ensure the record on file for fun club participation is current and complete with all required information.

Description: Based on observations by the LI, the program did not ensure all medications were kept in a locked place using a safe locking method that prevents access by children. Evidence: The LI observed a child's prescribed medication in the pharmacy container stored in the Center's first aid kit. The first aid kit was not in a locked place during the on site inspection and the first aid kit had also been taken with staff on the scheduled field trip.

Plan of Correction: The medication was locked in the storage closet at the time of the inspection. Staff will ensure all medications are kept in a locked box inaccessible to the children.

Description: Based on the LI's observations, a schedule of the field trip's events and locations were not posted and visible at the center site at the time the children and staff were out of the building on a field trip. Evidence: The LI conducted an unannounced monitoring inspection on April 2, 2015, the children were on a field trip to a local park, there was no posting of the center's schedule.

Plan of Correction: The Child Care Coordinator will ensure the schedule and location of any field trip is posted prior to leaving the Center.

Description: Based on documentation reviewed in a sample of children's records, parental permission for transportation and field trips was not secured before the scheduled activity. Evidence: 1. On inspection date April 2, 2015, the children were transported by the center on an outing to a local park. 2. The LI reviewed a sample of five children's records, documentation granting parental permission for transportation and field trips was not secured before the scheduled field trip on April 2, 2015. The center had implemented a blanket permission form for transportation and field trips. The blanket permission document covered the summer camp program for the year 2014. 3. The SACC Coordinator provided a copy to the LI of a notice that was sent to the children's parents which listed the upcoming scheduled field trips.

Plan of Correction: The (SACC) School Age Child Care Coordinator will ensure that each child's record contains a current written parental permission form prior to any center provided transportation and the child's participation in scheduled off-site field trips.
