Comments:
Today an unannnounced monitoring visit was conducted from 11:00am until 12:45pm. There were 15 children with 4 staff members. Children were observed playing on the playground, being fed, and napping. There are age-appropriate supplies in every room for the children to explore. Staff records, child records, evacuation drills, and playground equipment were inspected during this visit. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center's playground fencing was not maintained in an operable condition. Evidence: The fencing adjacent to the facility on the left side was no longer attached to the fence posts. The space provided would allow for a child to go between the fence and the post and could result in a hazardous situation.

Plan of Correction: The owner will get the fence fixed.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not keep hazardous substances in a locked place. Evidence: A bottle of dish detergent was observed on a table outside in the playground area while children were playing on the equipment.

Plan of Correction: We will make sure they are locked up.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center did not have protective coverings on all of the electrical outlets. Evidence: On the upper level the center had placed tape across two outlets. On the lower level the center did not have coverings on six outlets.

Plan of Correction: Will put new ones in the empty outlets.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interview the center did not meet the staff to children ratio as applicable to the youngest child in a group. Evidence: One staff was observed with 4 infants and 1 toddler in the infant room while the rest of the facility was on the playground. The program director stated that the toddler was unable to walk yet and so they were placed in the infant room for safety.

Plan of Correction: We will ensure another staff stays with the toddler to ensure ratio in the infant room.

Standard #: 22VAC40-185-500-B
Description: Based on direct observation the center did not maintain a nonabsorbent surface for diapering. Evidence: There is a small one inch tear on the diapering pad on the upper level of the facility.

Plan of Correction: Will buy a new diapering mat.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview the center did not have monthly evacuation drills documented. Evidence: The Evacuation Drill form's last documented drill was conducted 11/2014. The program director stated that the drills were conducted but may not have been written down on the form.

Plan of Correction: We will make sure that we document the drills.
