Technical Assistance:
Discussed the definition of and required information for a Volunteer. Medication Consent forms were also discussed. Risk Rating is provided on the Supplemental Page.

Comments:
An unannounced renewal inspection was conducted today from 10:00am-12:30pm. There were 34 (infants-5yrs.) directly supervised by 6 staff. The physical plant, 4 staff records, children? records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, playing inside, and doing art. Diapering and hand-washing procedures were also observed. There was a supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were checked and one group was out of compliance. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-70-A
Description: Based on record review, staff interviews, and observation the center has failed to maintain complete records for staff and for 2 other adults present and acting as volunteers in the center. Evidence: 1. The Director's file does not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. This staff's file also does not include documentation of experience to verify this staff member has experience required for the position. 2. During the inspection, the inspector noted two people helping in classes (two's group and four's group) and staff stated one was an observer and one was a parent volunteer who comes twice a week to help for a couple of hours. Both people were observed engaged in activities with the children. After an hour, staff stated the observer is actually an intern who is participating in activities. 3. The center does not have a file for either person observed that is required to contain name, address, verification of age requirement, job title, and date of volunteering; as well as the name, address and telephone number of a person to be notified in an emergency. The center also did not obtain two or more references on the volunteers to determine character and reputation as well as competency were checked before the two individuals began volunteering. 4. The Director was asked about the "intern" and she stated she had a letter from the Professor and other documents related to her internship but stated she was unable to locate it. She was then observed walking to the person asking for paperwork, took paper work to the office, made copies of the paperwork, and then gave the inspector the letter and information about the internship. 5. An interview with Volunteer A, revealed A has been at the center for a month interning from 830-1230 and that she has 2 more weeks of interning.

Description: Based on observation the center has failed to ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Inspector entered the preschool area in which there were 14 children (3 years-5 years) playing with 2 staff and an volunteer. In the room, there was a door ajar that lead to the teacher's lounge and staff bathroom. In the room, the Inspector noted a box on Borax Cleaner on a low shelf and a disinfectant spray bottle on a small table. 2. The maintenance closet is also accessed from this classroom and the door was unlocked. The closet had two shelving units right at the closets entrance and the inspector observed the two lower shelves were full with cleaning solutions, disinfectants, bug spray, and detergent.

Description: Based on observation and staff interviews, the center did not maintain ratio in the two's class when a 22 month old was observed in the class. Evidence: 1. There were 8 children present in the two's class with 2 adults. Upon interviews, it was determined one adult was not counted in ratio because she was not a staff member. 2. The Inspector asked how old the youngest child was in the group and the staff member noted child B was 22 months old and was transitioning into the two's group. 3. 22 months is a toddler and requires a ratio of 1 to 5.

Description: Based on record review and staff interviews, the center failed to obtain parental authorization to administer prescription medication before the center staff administered medication on two different occasions. Evidence: 1. Medication records show child A was given Albuterol by Center staff two times in the last 3 months. 2. The Center's Medication binder maintains a signed Medication Authorization form for Child A to receive Albuterol but it authorizes Fairfax County Staff to administer medication.

Standard #: 22VAC40-185-510-F
Description: Based on observation, the medication for Child A is not in the original container with the direction label attached. Evidence: 1. Medication for Child A is in a bag with vials of Albuterol but the prescription label is not kept with the medication to identify whom it is prescribed for.
