An unannounced monitoring inspection was conducted in the Center on April 14, 2015. There were 24 children present. As reported by staff, 30 children are currently enrolled in the program. The children were observed in two classrooms with 2 staff members supervising the children's activities in each room. The children were observed as they participated in varied activities. The staff members were observed interacting with the children and redirected as needed. The children did not participate in outdoor play on the date of the inspection due to rainy weather. The LI also observed the lunch meal and dismissal at the end of the program day. A sample of records was reviewed. Prescribed medications and written authorization to administer were reviewed for 6 children. It was verified four staff members have current medication administration training. Documentation of the program's annual fire and health inspections was current. Findings of the inspection were reviewed at the exit, if you have any questions, please call 204-9625. Description: Based on observations of the LI, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a locking method that prevents access by children. Evidence: The LI observed the key in the lock on the cabinet door under the sink in classroom A. Staff was observed removing containers of cleaning solution (soap/water) and sanitizing materials (bleach/water) from the cabinet for use on the lunch tables.

Plan of Correction: Policies and procedures were reviewed with all staff on the date of inspection. Another review will be conducted during the scheduled staff meeting.

Description: Based on observations of the LI, medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence: The LI observed child A.'s medication stored in the first aid kit in one of the classrooms. The first aid kit was out of reach of the children. The medication was in the original, labeled container loose in the first aid kit.

Plan of Correction: Policies and procedures were reviewed with staff. Medications will be stored in approved safety saks.

Standard #: 22VAC40-185-550-L
Description: Based on reviewed documentation, the Center did not notify the parent by the end of the day of any known minor injuries. Evidence: The LI reviewed the Center's accident/incident reports for the current program year, one reviewed form (child aa, dated 11/20/14, a bite mark) did not indicate that the parent was notified until the following day on 11/21/14, the time the parent was notified was not completed on the form.

Plan of Correction: Policies and procedures have been reviewed with staff. A review of the policies and procedures will be conducted at the next scheduled staff meeting.
