Comments:
Today an unannounced monitoring visit was conducted with the Director/Owner-Mrs. Ilisa. This visit took place in the morning, between 9:00-11 there were 2 classrooms observed with a total of 20 children with 5 staff within the supervision guidelines. The children were observed arriving to school, having morning free play, reading book, dramatic play, learning about space, puzzles and snack. Positive interactions were observed during this inspection. A complete inspection of the physical plant, staff records, fire drill log, menu and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on observation of the staff records, one staff record did not include documentation of two or more references were checked/verified, and orientation training. Evidence: Staff #1's record, did not include documentation that two or more references as to character and reputation as well as competency were checked before employment and documentation of orientation.

Plan of Correction: Director started to conducted the reference check however did not use the reference check form to document all of the required information. Will conduct and document.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the diapering station, the diaper changing pad was found no longer nonabsorbent. Evidence: The diaper changing pad was ripped/torn and no longer non absorbent.

Plan of Correction: Director will order a new one immediately.

Standard #: 22VAC40-185-560-G
Description: Based on observation of food brought from home, 5 lunch bags were found without the name and date identifying the owner. Evidence: 3 lunch bags did not have name and date.

Plan of Correction: Director added the names and dates during this inspection.
